What's the name of the gem I have to make, if I want to unlock this GGem?
Also, does Everlasting have effect on Flutes?

You need:
À¯¸®¿øÁÖx5, ÁøÇÑ ¾î·æÁ¾ÇÇx15, »ÏÁ·°©°¢Á¾Áý°Ô¹ßx15, °í±Þ ºñ·æÁ¾±¸½½x15
5x lapis lazuli jewel (the yellow glass jewel), 15x type 2 fish dragon blood, 15x type 2 crab claws, 15x type 2/advanced/awesome wyvern beads.
The monster items are all hr100+ monster thingies.

Yes you just need at least one of each of those, or the item just after the jewel itself. I don't remember how it works for those gems that need a jewel + items, where the gem + items requirement is that you just have one of the gems.
And everlasting affects only those things that you drink/eat. Flutes will be affected by flutemaster in breakage % and time.
